- Replace unpkg.com with cdn.jsdelivr.net in sw.js cache list
- Fix sw.js scope to /scanner/
- Remove unused $indexes parameter from checkAllIndexes()
- Extract duplicated " [%s] Index missing" literal to INDEX_MISSING_MSG constant
